I feel so embarassed for asking this , but
I just want to make sure I don't make any mistakes.........

1. Where EXACTLY can I find the UPC# on the DVD??

2. Should I put the FULL NAME of the DVD {Such as Special Edition}??
a. Do I call a Platinum Edition a Special Edition or a Platinum Edition??

3. What exactly should I write for the Make and Model of the DVD player, if it's a PS2??
a. Should I also write my regular DVD player's Make and Model as well??

1. On the back of the DVD on the right corner.
All the numbers

2. Full name call it Platinum Edition = Platinum Edition

3. If you have a DVD player put the model number it's on the front or your PS2.

and umm..........I would like to know if I should take out the Inserts in the DVD before sending it out??
